Independence and Wellbeing Service

What does the Independence & Wellbeing Service assist with?

The Independence and Wellbeing Team is responsible for assisting tenants to be able to live independently in their own home and managing their tenancy, health, and wellbeing. The team also welcome new tenants by undertaking an initial assessment to see what, if any, needs they have so that they can fully enjoy their tenancy– involving other agencies, when necessary, e.g., Age UK, etc. In addition to providing the community alarm service, the team will:

  • Visit you weekly/monthly/quarterly – to check that your general well-being is okay and agree on how we might help you further with your tenancy and living independently.
  • Ensure that all tenancy-related issues are in order and, if you have one, that your community alarm is working as it should be.
  • Undertake a quarterly estate inspection.
  • Undertake Tenancy Sustainment Plans and Goal Plans.
  • Carry out Keeping in Touch (KIT) visits.
  • Organise community activities you might like to participate in.
  • Encourage you to participate in activities that will take place at our Hubs.

The Independence and Wellbeing Team as trained Trusted Assessors: What does a Trusted Assessor do?

  1. Be able to assess the needs of an individual.
  2. Understand how low-level equipment and minor adaptations can assist with independence.
  3. Be able to work with an individual to implement solutions/adaptations.
  4. Be able to provide information about equipment and other solutions to suit an individual and their support needs.
  5. Understand the importance and relevance of policies and procedures for the equipment service.
  6. Be able to work in a safe, competent, and professional manner.
